Here are four designs of labels in PDF format that you can fill in in Adobe Acrobat Reader* and print out on your printer.

Procedure for good printing

If you have to fill in several identical labels, use copy/paste and the tab key to move from one field to the next.
In the "Print" dialogue, select "None" in the field "Scaling" so as to obtain the correct printing format.
